Requirements: Companies with 1 or more full or part-time employees must buy workers compensation insurance in Arizona

Sole Proprietors and Partners are automatically excluded from work comp insurance but Sole Props/partners can choose to be included on the Acord 130 or by endorsement.

Sole-Proprietors & Partners who opt to include themselves on a work comp insurance policy must use $31,200 payroll for premium calculations. (as of 01/01/2017)

Corporations –  Officers are automatically included in coverage, but they may elect to be excluded on the Acord 130 Form or by endorsement.

Corporate Officers who are not excluded from coverage must utilize a min payroll of $52,000 and a max payroll of $213,200 for the purpose of calculating workers comp premium. (as of 01/01/2016)

LLC – If the LLC is taxed as a corporation, LLC members are “officers” and capped accordingly unless they elect to be excluded. If an LLC is taxed as a partnership, LLC members are “partners.”  In turn, excluded from workers comp coverage unless the LLC Member elects to be included on the policy.
